Both medications can reduce appetite, support meaningful fat loss, and improve metabolic markers for eligible patients. Tirzepatide may produce greater average weight loss in clinical studies, but that does not automatically make it the right starting point for every person.

Both semaglutide and tirzepatide have been studied in adults with obesity or overweight with weight-related health conditions. In a major semaglutide study, participants without diabetes lost an average of about 15% of their body weight over 68 weeks when medication was paired with lifestyle intervention.

In a major tirzepatide study, average weight loss reached about 15% to 21% over 72 weeks, depending on the dose. A later head-to-head study also found greater average weight loss with tirzepatide than with semaglutide at 72 weeks in adults with obesity without diabetes.

Those numbers are encouraging, but averages are not guarantees. One person may respond exceptionally well to semaglutide and have fewer side effects, while another may need tirzepatide to see the appetite control and metabolic change they are looking for. Consistency, dose tolerance, medical conditions, and daily behaviors all influence the outcome.

FDA-approved brand names can be confusing

Semaglutide is the active ingredient in Wegovy for chronic weight management and Ozempic for type 2 diabetes. Tirzepatide is the active ingredient in Zepbound for chronic weight management and Mounjaro for type 2 diabetes. Side effects and safety considerations

The most common side effects of both medications are gastrointestinal. Nausea, constipation, diarrhea, vomiting, stomach discomfort, reflux, and reduced appetite can occur, especially during the first weeks or after a dose increase. Because tirzepatide can have a stronger appetite-suppressing effect for some people, it may also feel more intense during dose escalation. That does not mean semaglutide is automatically easier, since individual tolerability varies widely.

More serious risks are less common but require real medical oversight. These medications may not be appropriate for people with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2.
