Your Annual Wellness Visit Checklist for 2026

Your Annual Wellness Visit Checklist for 2026

A good annual visit should do more than produce a normal blood pressure reading and a reminder to exercise. It should give you a clearer picture of where your health stands, what may need attention, and what to do next. This annual wellness visit checklist helps you arrive prepared, use your appointment time well, and leave with a plan that fits your life.

The details of a preventive visit vary by age, medical history, sex assigned at birth, family history, current medications, and personal goals. A 28-year-old with no chronic conditions will not need the same conversation or screening schedule as a 55-year-old managing high blood pressure, weight changes, and a family history of colon cancer. That is why preparation matters: it helps your clinician focus on the care that is right for you.

Start With the Type of Visit You Are Scheduling

The phrase “annual wellness visit” can mean different things. For many adults, it refers to a yearly preventive checkup with a primary care clinician. If you have Medicare, the Annual Wellness Visit is a specific benefit focused on prevention, health risk assessment, and a personalized prevention plan. It may not include a hands-on physical exam or address every new concern in the same way as a routine problem visit.

When scheduling, ask what the appointment includes and whether you should book additional time for ongoing symptoms, medication changes, or chronic-condition follow-up. Preventive services are often covered differently than care for a new medical problem, and insurance rules can vary. Clear expectations help avoid surprises and make sure you get the right kind of appointment.

Your Annual Wellness Visit Checklist Before the Appointment

Preparation does not need to be complicated. Set aside 15 minutes a few days before your visit to gather the information your clinician will use to guide recommendations.

Update your health information

Bring your insurance card, photo ID, and any forms your office asked you to complete. If you have seen another clinician, visited an urgent care center, received hospital care, or had testing done since your last visit, write down where and when. Records do not always transfer automatically between health systems.

Also update practical details that shape care, including your preferred pharmacy, emergency contact, and any changes to your address or contact information. For telehealth appointments, confirm that you can access the patient portal and have a reliable internet connection before your scheduled time.

Make a complete medication and supplement list

Include prescription medications, over-the-counter medicines, vitamins, herbal products, injections, and as-needed medications. For each one, list the dose, how often you take it, why you take it, and whether you have noticed side effects or trouble affording it.

Medication review is especially useful if you take several prescriptions, have more than one prescriber, or have started a new wellness or specialty treatment. Your clinician can check for duplications, interactions, opportunities to simplify your routine, and refills you may need.

Write down changes since your last visit

Small changes can be clinically meaningful when they persist. Note new symptoms, changes in sleep, energy, mood, appetite, weight, digestion, menstrual cycles, sexual health, breathing, pain, or exercise tolerance. Include when each issue started, what makes it better or worse, and how it affects daily life.

Be direct about alcohol, tobacco, vaping, recreational drug use, stress, and mental health. These are health details, not a test of character. Honest answers allow your care team to recommend support that is realistic and safe.

Check your family health history

Tell your clinician about close relatives with conditions such as heart disease, high cholesterol, stroke, diabetes, cancer, osteoporosis, or mental health concerns. A new diagnosis in a parent, sibling, or child may change when certain screenings should begin.

If you do not know your family history, say so. Many people do not have access to complete information, and your clinician can still build an appropriate preventive care plan.

What to Discuss During Your Visit

Annual care works best as a conversation, not a checklist completed around you. Your clinician may review your vital signs, medical history, lifestyle, medications, and recommended preventive services. They will also consider what matters most to you.

Preventive screenings and vaccines

Ask which screenings are due now, which can wait, and why. Depending on your age and risk factors, this may include blood pressure checks, cholesterol testing, diabetes screening, colorectal cancer screening, cervical cancer screening, breast cancer screening, lung cancer screening, bone density testing, or screening for infections such as hepatitis C or HIV.

Screening recommendations are not one-size-fits-all. For example, family history, smoking history, pregnancy plans, sexual activity, prior test results, and chronic conditions can affect the timing and type of screening. Your clinician can explain the benefit of a test as well as its limitations, including when a test may lead to additional follow-up.

Review your immunizations too. Flu, COVID-19, tetanus, shingles, pneumonia, hepatitis, and HPV vaccines may be relevant depending on your age, health conditions, work, travel, and vaccination history. If you are unsure what you have received, your clinician can help you determine what records are needed.

Your daily health habits and realistic goals

A wellness visit is a useful place to discuss nutrition, movement, sleep, stress, and weight without treating any of them as a moral failing. Focus on patterns and barriers. If you are working long hours, caring for children, managing pain, or struggling with sleep, a plan that requires an hour at the gym every day may not be practical.

Choose one or two outcomes that would make the biggest difference over the next few months. That might mean bringing blood pressure readings into range, walking after dinner three days a week, improving sleep consistency, reducing alcohol use, or discussing structured weight-management support. Specific goals are easier to revisit than a vague promise to “be healthier.”

Chronic conditions, symptoms, and specialist care

If you live with diabetes, asthma, hypertension, thyroid disease, anxiety, depression, high cholesterol, or another ongoing condition, ask how well your current plan is working. Bring home readings if you track blood pressure, blood sugar, peak flow, weight, or symptoms. The pattern over time often tells your clinician more than a single measurement.

Use the visit to clarify who is managing each part of your care. A primary care clinician can monitor routine needs, adjust appropriate treatment, order follow-up testing, and coordinate referrals when specialty care is needed. If you already see specialists, ask how results and treatment updates will be shared so important details do not get lost between appointments.

Questions Worth Asking Before You Leave

You do not need a long list of questions. A few focused questions can make your next steps much clearer:

  • Which preventive screenings or vaccines are due for me, and which are optional based on my risk?
  • What do my vital signs and lab results mean in the context of my overall health?
  • Is there anything in my medications, supplements, or lifestyle that needs to change?
  • What should I monitor at home, and what numbers or symptoms should prompt me to contact you?
  • When should we follow up, and do I need a referral or additional testing?

If something is unclear, ask your clinician to explain it in plain language. You should know what is being ordered, why it is being recommended, how to prepare, and how you will receive results.

Turn the Visit Into Ongoing Care

The appointment itself is only one point in your care. Schedule recommended testing promptly, pick up prescriptions, and add follow-up dates to your calendar before the details fade. If you receive abnormal results, do not assume the worst, but do not ignore them either. Ask what the result means, whether it needs to be repeated, and what the next decision point will be.

For people balancing work, family, and ongoing health needs, virtual primary care can make regular monitoring easier to maintain. My Healing 365 provides membership-based primary care with licensed clinicians who can support preventive planning, routine monitoring, chronic-condition care, prescription needs, and coordination with specialists when appropriate.

Your health plan does not have to be perfect to be useful. Bring the information you have, name the concerns you have been putting off, and take the next recommended step. That is how an annual visit becomes care that supports you all year long.
